You are almost there.  Session 5!!! What an accomplishment.  We are very proud of you.   In the back of your CDA Competency Standards Book, you will find your CDA Application and Comprehensive Scoring Instrument.  You will complete the CDA Application (DO NOT include any credit card information) and email it to [email protected] or fax it to 678-369-4857.  DO NOT write on the Comprehensive Scoring Instrument.  Please place this 21-page document in your empty 3" Binder so that it may be given directly to your PD Specialist on the day of your CDA Assessment.
