This is the CDA registration module. The information from this module will help us to guide you. Once you complete the information, we will put you into your first paid session. This payment covers the cost of the first 20 hour session only. For the Pay As You Go program, each module must be paid for individually.  2. The student is responsible for purchasing the textbook from the CDA Council, required materials and cost of the assessment with the CDA council. More details will be provided. 3. You are almost there.  Session 5!!! What an accomplishment.  We are very proud of you.   In the back of your CDA Competency Standards Book, you will find your CDA Application and Comprehensive Scoring Instrument.  You will complete the CDA Application (DO NOT include any credit card information) and email it to wellablecda@gmail.com or fax it to 678-369-4857.  DO NOT write on the Comprehensive Scoring Instrument.  Please place this 21-page document in your empty 3" Binder so that it may be given directly to your PD Specialist on the day of your CDA Assessment.
